All new staff who joining to J&T Express (Malaysia) Sdn. Bhd shall complete all the information required in the Form upon entering to Company's premises. This form shall be filled in completely and submitted to Human Resource Department (HRD)

Health care associated exposure, including providing direct care for COVID-19 patients, working with health care workers infected with COVID-19, visiting patients or staying in the same close environment of a COVID-19 patient.
Working together in close proximity or sharing the same classroom environment with a with COVID-19 patient
Traveling together with COVID-19 patient in any kind of conveyance
Living in the same household as a COVID-19 patient

Declaration and Data Privacy Consent Form

By signing this HEALTH DECLARATION FORM, I declare all the above statement are true and correct at the time of signing. I further declare that I do not have any of the symptoms associated with the COVID-19 or similar upon return this form. I truly understand that there is serious offence for provide false or misleading statement and information in accordance with government laws and regulations. J&T Express (Malaysia) Sdn Bhd has reserved absolutely right to take necessary legal action against me if I provide any false or misleading statement and information in the form.
