MEMORIAL FUND REQUEST
The Memorial Fund Committee meets quarterly, requests can be submitted at any time. Submitted forms will come to Pastor Lauren for committee consideration. Upon approval, contact person will be notified via an approved form. The approved form will need to be submitted along with receipts to Terri and Pastor Lauren for payment/reimbursement.
